Acute epiglottitis (supraglottitis) is a dramatic and potentially lethal condition occurring in children between 2 - 7 years with peak incidence at about 3.5 years. It is seen much less commonly nowadays due to wide spread immunization against Haemophilus influenzae type b. It is characterized by a fulminating course of high fever, sore throat, dyspnea, rapidly progressive respiratory obstruction, and prostration. It may progress to complete obstruction of airway and death within a matter of hours, unless adequate treatment is provided. Because most patients have bacteriemia, intravenous antibiotic therapy with cefotaxime, ceftriaxone or ampicillin + sulbactam should be started promptly.
