Pregnant women who do not have GBS screening results at the time of labor and delivery, intrapartum antibiotic prophylaxis should be given to women EXCEPT
a) Who are <37 weeks gestation
b) Who have a duration of membrane rupture ≥18 hours
c) Who have a temperature of ≥100.4º F (≥38.0ºC)
d) All women irrespective of membrane rupture
